Y - HLTH is an innovative application that facilitates communication with the doctor and the patient. Y-HLTH is database of medical history for patients, shared medications, mood tracker records. Doctor can have overview of medical history in certain period of time, patients can share unused medications, choose personal doctors and share medical histories.

Major functionality of application are:

Medical history – user have opportunity to add his medical history a PNG, JPEG, PDF format and can share his medical history with the doctors.

My Doctors – patients are given the opportunity to choose a doctor from the internal database or to send a request to your doctor added it to the base doctor. The patient can add a doctor who is not registered in the database. The doctor will arrive email to register, so that each patient can choose their personal physician. Doctors can inscribe the base of your personal information, education, awards and previous work experience.

Share medical history - the patient can be divided your medical history with a doctor who was added in My Doctors so that the doctor can have access to his medical records.
